Fees and Mailing Instructions
Parents can mail completed form and fees to:

Forms to Include:
- Completed and notarized AOP (original copy)
- Child’s certified birth certificate (vital records charges an additional fee if a certified birth certificate is not included)
- If a 3-party AOP was completed: a certified DNA-based paternity test that demonstrates paternity with at least 99.9% probability
Fees:
- Check or money order for $27.50 payable to “Louisiana Vital Records” (DO NOT MAIL CASH).
- This fee includes one certified copy of the child’s amended birth certificate.
- At the time of submission, parents may also include $9.00 for each additional certified copy they would like to receive.
- I f they do not submit a certified copy of their child’s birth certificate, they must include an additional $15 to cover a statutory search.
- Fees for walk-in-services at the Central Vital Records Office in New Orleans are payable by cash, check, or money order.
