All fees are to be paid by certified check or money order made payable to the Department of Children & Family Services. Do NOT send cash, business or personal checks.

- 4-6 beds – $ 400
- 7-15 beds – $ 500
- 16 or more beds – $ 600
- Child Placing Agency – $50
- Maternity Home – $50
