Submit this completed application form through one of the following methods:

Online: Submit directly from the DCFS Website by selecting submit.
In Person: Visit your child support office at 3519 Patrick Street, Lake Charles, LA 70605 from 8 a.m. to 4:30 p.m., Monday through Friday. This program is only available in the Lake Charles Region.

I understand that the Child Support Enforcement staff will contact me to determine my eligibility for enrollment into this employment and training program. If eligible, I will be referred to the LIVE program and then the LIVE staff will contact me regarding enrollment and will provide intensive case management throughout my enrollment in the program.
