Annual Care Facility Licensing Fees

All fees are to be paid by certified check or money order made payable to the Department of Children & Family Services.  Do NOT send cash, business or personal checks.

• Residential Home
  
  • 4-6 beds - $ 400
  • 7-15 beds - $ 500
  • 16 or more beds - $ 600
• Child Placing Agency - $ 50
• Maternity Home - $50
